Our clients fall into three sections. about 30% want to choose a theme and then have it implemented, modules written to enhance the functionality, upload and support. Some 30% want us to code against provided PSD and wireframe, generally with a reasonably good specification. 30% want us to develop a theme and then install and support it, all from zero and, typically, 10% come to us with absolutely no idea what they want and trust us to advise them and depend on us to provide them with a good solution at a fair price. We have never lost a client!
